Out personalized Palliative Care Services are available around the clock!Dealing with illnesses are challenging, but dealing with the transition from a health facility to home can be both confusing and stressful. The care your loved one receives during this time is critical to ensure all necessary instructions and safety measures are taken so they do not end back up in the hospital.
Our specially trained Transitional Caregivers will meet you and your loved one at the facility and will guide you through the entire hospital-to-home process. We will make sure all of the new medications prescriptions are filled, follow up appointments are made and transportation arrangements have been coordinated. Nutritious meals and beverages are readily available upon arrival home and all personal care needs are provided for.
Most important is that we are there to provide you and your loved one support and encouragement throughout the entire process to ease unnecessary stress. Our Post Discharge Care Services will ensure your loved one receives all of the proper care immediately upon coming home to prevent unnecessary re-hospitalizations. We are passionate about providing the very best care to our seniors. Geriatric Care Management Services are available to our clients that have greater care needs to remain safely in their home. This comprehensive service includes complete management of care for your loved one.
Services include being assigned a personal Geriatric Care Manager with multiple years of experience managing geriatric clients. Our Care Managers will thoroughly evaluate your loved one for all of their physical, behavioral, financial, and social needs and then coordinate services and resources as needed. We then monitor each patient’s progress daily, weekly or monthly as needed and take care of any additional needs on-going.
A monthly report and status update will be completed and sent to you via e-mail, phone or via the Family Room. Out Geriatric Care Managers are available to our patient’s 24-hours a day to provide you with peace of mind that they are in safe hands at all times.
