During your initial consultation, Dr. Loxley will request X-rays. She may prescribe antibiotics or anti-inflammatory medication to take prior to returning for surgery.When you return for surgical root canal, the area will be numbed with a strong amount of local anesthetic.

Dr. Loxley will cut the gum and move it away from the tooth for easier access to the root. She may have to cut away some of the jaw bone to expose the root tip. Infected surrounding tissues and the edge of the root tip are then removed. After cleaning the canal, filling is placed in it and at the end of the trimmed root. The gum is repositioned and stitched into place.

The location and type of the tooth determines the amount of time an endodontist needs to complete an apicoectomy. Allow 30 to 90 minutes for that part of the process. Dr. Loxley will set a follow-up appointment. If the surgical root canal therapy is successful, the infection will go away very quickly. You will be advised that the bone tissue around the root tip will take a few months to fully heal.

In some cases the infection continues after surgery and the tooth will need to be extracted to prevent damage to other teeth, your jaw, and gums. The problem may be due to a root fracture, which cannot always be detected until after the tooth is pulled or because the pain was ignored quite a while before seeking treatment.

Your teeth may also be cleaned before determining whether non-surgical or standard surgical root canal therapy is needed.

How similar is either type of root canal procedure to extreme fillings? Dr. Loxley or her staff may compare the non-surgical root canal process to getting a very deep filling. A filling involves the doctor or her assistant applying local anesthetic to reduce pain while she is working on your tooth. Once the tooth and surrounding area are sufficiently numb, the doctor drills into the affected tooth, cleans out the diseased area, and fills the hole to prevent additional infection.

Non-surgical root canal therapy, also called non-surgical Endodontic therapy, requires deeper drilling. The entrance through the tooth provides Dr. Loxley access to the root canal where the tooth’s pulp and nerves are located. After the inflamed and infected pulp is removed, the doctor disinfects and thoroughly cleans the inside of the tooth. Then it is filled and sealed with gutta-percha, a material closely resembling the texture of rubber. The tooth is protected by a filling or crown.

Both non-surgical and surgical root canal treatment are usually completed in two appointments. In the case of a severe infection or the reason for the pain is not visible in an X-ray, Dr. Loxley will recommend surgical root canal. The area is numbed and an incision made in the gum. The doctor will access the canal from the side to remove the infected tissue.
